Thank you for your interest in enrolling your child at Sacred Heart School, Mundaring.

We are a single stream school catering for students from Kindergarten to Year 6.

For enrolment at Sacred Heart School, download and complete the Application for Enrolment form for each child enrolling.

The following documents must be submitted with application/s.

Birth Certificate
Immunisation History Statement (from MyGov/Medicare)
- Department of Health – Immunisation History
Baptism Certificate (if Catholic)
Parish Priest Reference Form (if Catholic)
Data Collection Form
If your child is born outside of Australia, we require a copy of your child’s current passport and a copy of the visa details or alternatively a copy of your Australian Citizenship.

There is NO APPLICATION FEE.
