Thank you for your interest in enrolling your child at Sacred Heart School, Mundaring.
We are a single stream school catering for students from Kindergarten to Year 6.
For enrolment at Sacred Heart School, download and complete the Application for Enrolment form for each child enrolling.
The following documents must be submitted with application/s.
- Birth Certificate
- Immunisation History Statement (from MyGov/Medicare)
- Baptism Certificate (if Catholic)
- Parish Priest Reference Form (if Catholic)
- Data Collection Form
- If your child is born outside of Australia, we require a copy of your child’s current passport and a copy of the visa details or alternatively a copy of your Australian Citizenship.
Please submit the application form/s to the school office together with the non-refundable Application Fee of $30 (payable per application).
